Tailored Support for a Life of Possibility

Our Support Model

Attainment Care will offer supported living services tailored to adults aged 16+ with:

  • Learning disabilities
  • Autism and neurodivergence
  • Mental health support needs
  • Behaviours that challenge
  • Complex or forensic risk profiles

Every individual will receive bespoke, person-centred support in safe, well-matched homes — designed to promote stability, wellbeing, and independence.

We follow SCIE’s definition of supported living: separating tenancy and care, building independence, and offering people control over their environment and support.

Learning Disability & Autism Support

Our experienced team works with people with a wide range of abilities, communication styles, and support needs. We create safe, empowering environments where individuals feel secure, understood, and respected.

Our autism support is underpinned by the SCIE ‘Supporting autistic people’ guidelines and CQC’s  “Right Support, Right Care, Right Culture” framework.

Person-Centred Planning

We use outcome-focused support plans to help people:

– Set and reach their personal goals

– Build life skills (e.g. cooking, shopping, money management)

– Engage with their local community

– Maintain routines that work for them

We use SCIE’s Co-production model to ensure people are not just heard—they are central to every care decision.
